LANL Privacy Statement:

Triad National Security, LLC ("TRIAD"), Management and Operating Contractor for the Los Alamos National Laboratory ("LANL"), is requesting that you provide your Personality Identifiable Information (PII) in order to process your request for security access authorization with the Department of Energy/National Nuclear Security Administration ("DOE/NNSA"), in performance of it Contract No. 89233218CNA000001 ("Prime Contract") with DOE/NNSA. This information, including your Social Security Number if requested, is being collected as authorized by 42 U.S.C 7101 et seq., 50 U.S.C. 2401 et seq., and Executive Orders 12333, 12958, 12968. An individual's name and Social Security Number are used as identifying factors to establish and maintain records of DOE access authorization actions. This information will be used and disclosed by DOE/NNSA, TRIAD personnel, and subcontractors or other agents who need the information in the performance of their duties under law and TRIAD's Prime Contract with DOE/NNSA to process and/or determine eligibility for security access clearance. Your furnishing this information is voluntary; however, failure to furnish this information may result in a delay in (or denial of) processing this request for the individual's access authorization/security clearance. As the information you provide will be incorporated into LANL and DOE/NNSA files for ongoing use, you must ensure that it is accurate and that you will provide updates if there are any changes. By submitting your PII to TRIAD, you consent to the collection and use of information by TRIAD as indicated above.
